This form will include tips and hints on how to answer the questions, which will help DietPro Solution provide you with recommendations suitable to your unique lifestyle. Information collected on this form includes details of:
- Anthropometry (measurements such as weight, height and waist circumference)
- Medical history
- Lifestyle and social information
- Diet history
Once you return this form to DietPro Solution, we will assess the information provided and offer you practical recommendations and a realistic plan of action to improve your nutritional status, including supporting education resources. You will then have the option of registering for email review consultations, in which we will reassess the dietary and lifestyle changes you have made.
